This week will contain general information about health promotion and disease prevention along with specific material related to a vulnerable population that was chosen in Week 1. The content of the paper will be developed through the first 8 weeks of the course. This allows students to take from the course work and learning assignments to complete the paper.

                                          i.         Introduce your vulnerable population – Sinner or Victim?

                                        ii.         Social justice related to your population (DeChesnay, Ch. 4)

                                       iii.         Low literacy within your population and how this affects their health (DeChesnay, Ch. 5)

                                      iv.         Choose a Nursing model or theory and a midrange theory that you believe will best relate to your vulnerable population and health promotion

1.    The theory will come from Chapter 5.

2.    The mid-range theory for application will come from Chapter 6 (DeCehsnay, Ch. 5-8; Pender, Ch. 2 – – you should have completed this for your Week 2 assignment)

                                        v.         Evaluate your vulnerable population related to Healthy People 2020 guidelines

                                      vi.         Discuss 3 primary health concerns for your population and include preventative guidelines as appropriate

                                     vii.         Consider the community in which your vulnerable populations live and what type of assessment you might need to conduct? In real life this is lengthy and complicated, but for the purpose of the paper – the goal is to give a brief overview of the how you would initiate the process and what you might find either figuratively or in your own community. (Pender Chapter 4)

                                   viii.         Develop a health promotion opportunity and health plan to address ONE of these concerns specific to your Case Scenario – see below (DeChesnay, Ch. 5; Pender, Ch. 4 & 5; Saunders article, p.138)

1.    Provide a Case Scenario describing an individual from your vulnerable population and 1-2 individuals that would be considered a support system. Based on the scenario created, develop a Health Promotion Opportunity and Prevention Health Plan (Pender, Ch. 5).

2.    The following is a case scenario example: Denise is a 17 y.o. Caucasian female who is a runaway and 28 weeks pregnant. She drank alcohol on the weekends until she found out she was pregnant; she smokes when cigarettes are available, and she has smoked THC 5-6 times over the past few months. Denise spends most of her nights in a shelter in Seattle and her days hanging out and panhandling. Denise’s “BFF” is Low-Rider. He’s a 43 y.o. Hispanic male who has been on the streets of Seattle for about 3 months after he hitched a ride from Austin TX. Sara Ann is a local 30 something y.o. female who volunteers at the soup kitchen that Denise likes to talk to. Denise had 3 months of initial prenatal care in Omaha Nebraska but her step-dad is an alcoholic and pushed her down some stairs when she tried to stop him from hitting her Mom a few weeks ago. It all ended in a big blow up “as usual” and Denise packed what she could in a backpack and took off – lucky for her she found a ride with a trucker who was heading west.

1.    What support system did you determine to be appropriate for your scenario?

2.    What kind of emotional support diagram does your vulnerable population person have?

3.    The health promotion opportunity should cover the 9 steps of the planning process and all three aspects outlined within the Saunders article: environmental component, curriculum component, and training component.

4.    The health promotion opportunity should contain aspects of primary, secondary and tertiary prevention.

5.    The health promotion opportunity should be feasible for you to conduct in your community and require only limited financial funding.

6.    The health promotion opportunity should reflect content from preventative guidelines and Healthy People 2020.

7.    From the health promotion opportunity create a flyer/poster to circulate inviting your vulnerable population to the health promotion outreach. Within your paper, describe the content of the flyer and locations you would post the flyer to gain the most exposure. The flyer should be included in your paper as an attachment per APA formatting rules. Started in Week 3 and Due in Week 6 of the Course.

8.    The health prevention plan should be based on the nine steps discussed in Pender, Ch. 5. Using this process, develop a health plan for one individual from your vulnerable case scenario. Be sure to clearly identify the nine steps in the health planning process while being specific to the individual within your case scenario.

                                      ix.         Discuss use of Community Based Participatory Research (CBPR) with your vulnerable population – pros and cons (DeChesnay, Ch. 15)

                                        x.         Discuss your brief plan for your community assessment (Pender Ch. 4)

                                      xi.         Discuss how vulnerable populations are to be protected when participating in research, free programs, programs that pay stipends for participation etc., and why this is so important. (DeChesnay, Ch. 11)

 

d.    Section 4

1.    The conclusion of your paper should bring all your information together.

2.    Discuss research articles, evidenced based practice information related to your vulnerable population group.

3.    Review community outreach resources that are available in the area in which you believe you would find the highest concentration of your particular vulnerable population.

4.    REACH: Racial and Ethnic Approaches to Community Health – Evaluate the information for usefulness related to your Vulnerable Population.

5.    Discuss what resources are available within your community for your specific population and vulnerable populations in general.

6.    Discuss how as an APRN you can impact vulnerable individuals in your community.

7.    Conclude with a summary of how we as a nation are doing with health promotion, disease prevention and Healthy People 2020.

8.    What is a topic that you think should be included in the Healthy People 2030 guidelines, and why?
